Just fill out the form at any of our stores, get your card, and we’ll do the rest. Right now cards are only available at the store.  

Once you have your card, remember to use it every time you  make a purchase. 

Just hand your card to the cashier when you check out to receive points for your purchases. We cannot enter your purchases for rewards after a transaction is complete.

We issue coupons for the each month's purchases at the end of the following month. For example, any points earned in February will be reflected in coupons issued at the end of March. The coupons are all good for 60 days from when they are issued.

The amount of your coupon will depend on how many points you've earned in the month. Coupons are issued based on 10 point increments--$5 for every 10 points earned. If you've got extra points, they will roll over to the next coupon.

Rewards are subject to the terms and conditions printed on the card. Points are earned on purchases net of   discounts and before tax. We will use your email and mailing address to send you our Pharm Life newsletter and coupons. If you have a newsletter mailing preference, please let us know (coupons must be mailed). We will NEVER  sell your information to another company without your permission.
